When we receive your referral from your doctor, we will contact you with an appointment promptly
On the day of your appointment please remember to bring along a CD or hard copy films of your MRI and/or other imaging (and report if available). If your films cannot be viewed on the day of your appointment, you may have to be rescheduled
You can download the registration form here to bring in on the day
Please notify us as soon as possible if you are unable to make your appointment
Failure to attend appointments may incur a fee
